I accept Health Savings Account cards, debit cards, credit cards from the four major credit card banks, checks, cash, and money orders. I typically bill insurance $140.00 for a 50-60 minute individual clinical hour. Cash paying clients may be offered a discount, depending on their individual circumstances and at my discretion.<\/p>\n\n\n\n<p>I am an \u201cin network\u201d credentialed provider with the following insurance companies:<\/p>\n\n\n\n<ul><li>Premera Blue Cross<\/li><li>Regence Uniform<\/li><li>Blue Cross Blue Shield<\/li><li>First Choice Health<\/li><li>Lifewise of Washington<\/li><li>Providence Behavioral Health (PBH)<\/li><li>Kaiser Permanente (First Choice Health options)<\/li><\/ul>\n\n\n\n<p>Please call your insurance provider to determine other out of network options, or call my office if you have further questions concerning insurance billing.<\/p>\n\n\n\n<p>I require 24 hour notice to reschedule or cancel any therapy appointment; \u201cno shows\u201d and missed appointments are billed at the full clinical rate. If you are an adult and someone else is paying for your therapy, I request that you pay me directly and your family member etc. reimburse you for services. I routinely provide billing receipts upon request for tax purposes etc. It is expected that children under the age of 18 would have a parent or guardian pay for their services.<\/p>\n\n\n\n<p>Clients requesting professional letters will be billed an hourly rate for such services. <strong><em>It is up to the discretion of the clinician whether or not such letters are appropriate.<\/em><\/strong> Letters may include but are not limited to: FMLA letters, medical\/mental health disability letters, employee leave letters, student hardship letters, pet therapy letters, court letters\/declarations, physician letters, short term\/long term mental health disability leave letters, and therapy enrolment letters.<\/p>\n\n\n\n<p class=\"has-text-align-center\"><a href=\"http:\/\/dawnstremel.com\/updated\/counseling\/contact-me-about-counseling-request-an-appointment\/\"><strong>Contact me for therapy services.<\/strong><\/a><\/p>\n\n<\/div><\/div>\n<\/div><\/div>\n<\/div><\/div>\n\n\n<p><\/p>\n","protected":false},"excerpt":{"rendered":"<p>All fees are due at the time of service, including unmet insurance deductibles and co-pays.
